Employee Benefits

The City of Ames offers a comprehensive benefits package for employees regularly working 20 hours or more per week. Benefits are pro-rated for employees working between 20-39 hours per week. The benefits available include vacation, sick leave, family sick leave, emergency leave, life insurance and AD&D, health and dental insurance, vision insurance, retirement, long-term disability, tuition reimbursement, and an employee assistance program. 1095-C Distribution Notice

As an employer sponsoring a group health plan, we provide Form 1095-C for full-time employees.  As a self-insured group health plan we also provide a form for non-full-time employees and other individuals covered on our plans. 

Form 1095-C is an information document used to help administer certain provisions of the Affordable Care Act (ACA).  You do not need the form to file your tax returns.  For more information, please visit the Internal Revenue Service's Q&A External Site
