City of Ames Utility Requests

Below is a general utility request form. It includes the ability to start or stop services, sign up for e-notifications, sign up for auto pay, and more.

 

While filling out the form below, please be on the lookout for REQUIRED FIELDS. These are the only fields needed for form submission.
